NO! The original OBI is not included. I bought this brand new years ago, and didn't even know I needed to keep it. 
Anyway, I would call my copy very good condition. In fact, the disc itself and paper contents are nearly perfect. The plastic case has some shelf wear, minimal I guess. The picture below is the copy I have, again, minus the OBI sleeve.
Since I don't have the OBI, I'll be selling my copy to lower US 48, Paypal.
