I just found this, evidently, a jacket was prepared, it'd be interesting to find out why the CD-4 release was cancelled 50 years ago.

Making it that far to actual release it seems very strange that it was cancelled. I wonder if there were any issues with the CD-4 pressing?
It would be fantastic if someone involved in the Warner groups quad program back then could weigh in and give us the back story.

We have to thank ForagingRhino for unearthing these Quadios, especially the previously unreleased ones like this!
