Just discovered another one for your list, viz. Peter Cobbin's 5.1 mix for the 1999 DVD and 2012 DVD & Blu-Ray releases of The Beatles' Yellow Submarine. I picked up the Blu Ray a year or so ago and was curious as to why there were no audio credits. A post on the Steve Hoffman forums led me to this:

--and there seems to be a consensus that while the film itself was cleaned up considerably for the 2012 release, the soundtrack was untouched from the earlier one (albeit upgraded to DTS-MAHD).
