I was wrong. All mch tracks are affected.
After fixing a password on an email account I see that Roadrunner emailed me on 9/11/2014 as well.
Their response on the Redemption page was "to be sure I was on this page: http://redemptions.warnermusic.com/opeth-blu-ray [printed card URL is: http://redemptions.warnermusic.com/opeth ]
Using that link on my phone, it worked and went through this time.

Hey- I want to order this, but am unaware if Amazon.com has the corrected disc yet. Also checking Burning Shed for them to put it back up for sale. ???anyone??? Thanks.
No idea. have tried to find out from Amazon but they were evasive and I think they don't have it in yet. I've also emailed Roadrunner records and have yet to get a reply.
Everything points to corrected discs on or around Sept 30th; chill for a couple weeks.
UPDATE: UPS just delivered a replacement Blu Ray disc here (Next Day Air service)! It came from CINRAM MFG in Olyphant, PA (which has been a Warner disc plant for years?)
I originally purchased from Roadrunner directly so I cannot comment on the velocity of replacement for other purchases made elsewhere.
NOTE: The disc is in a plain white sleeve and the artwork is IDENTICAL (still hard to read); this disc appears to me to be a lighter shade but not much. Could just be duplication anomaly.
Relief is on it's way.

Yes I did. Probably doesn't matter when submitted as the replacement plan was already in motion.
